The Galveston County Health Information Exchange and the Health Information Exchange of Southeast Texas are merging with reater Houston Healthconnect. GCHIE is comprised of two hospitals and 752 physicians and HIESET includes 16 hospitals and 650 physicians in six counties. Healthconnect will noe included ~133 hospitals and over 14,000 physicians over a 20 county area.

A broad set of stakeholders in South Texas are using the Direct Project to improve the health status of persons in South Texas with diabetes, including gestational diabetes. Participants come from the medical community (CHRISTUS Health, the Health Information Network of South Texas, the Driscoll Children’s Health Plan, Corpus Christi Medical Center, the Public Health Department, and Nueces County Medical Society), community-based social service organizations, colleges, and employers (the Coastal Bend Diabetes Community Collaborative, The Salvation Army, United Way, and others).
